FAKTOR FAKTOR YANG BERHUBUNGAN DENGAN KERUSAKAN BERKAS REKAM MEDIS RAWAT JALAN DI PUSKESMAS PLUS PERBAUNGAN KABUPATEN SERDANG BEDAGAI TAHUN 2026

The results showed that the majority of intrinsic factors were in the good category, accounting for 74 files (80.4%), the majority of extrinsic factors were in the non-standard category, accounting for 48
files (52.2%), and the majority of medical record files were not damaged, accounting for 66 files (71.7%). The Chi-Square test results showed a significant relationship between intrinsic factors and damage to outpatient medical record files (p-value = 0.000 < α 0.05), and a significant relationship between extrinsic factors and damage to outpatient medical record files (p-value = 0.000 < α 0.05). Therefore, intrinsic and extrinsic factors were associated with damage to outpatient medical record files at Perbaungan Plus Public Health Center, Serdang Bedagai Regency in 2026. It is recommended that the Perbaungan Plus Public Health Center improve the management and maintenance of medical record storage facilities to prevent damage to medical record files.
