The study sample was 34 preoperative patients selected using a total sampling technique. Anxiety levels were measured before and after the implementation of therapeutic communication using the Hamilton Anxiety Rating Scale (HARS) questionnaire. Data analysis began with the Shapiro-Wilk normality test and continued with the Wilcoxon Signed Rank Test with a significance level of α = 0.05. Results: The results showed that before the implementation of therapeutic communication, the majority of respondents experienced moderate and severe anxiety. After the implementation of therapeutic communication, the patient's anxiety level decreased, where most respondents
were in the mild anxiety category. The results of the Wilcoxon Signed Rank Test obtained a p-value = 0.000 (p <0.05), which indicates a significant difference between anxiety levels before and after therapeutic communication. Conclusion: Therapeutic communication significantly reduced anxiety levels in preoperative patients at Grandmed Hospital, Lubuk Pakam, in 2025.
