The degree of diabetic ulcers was classified using Grade 1 to Grade 5 assessment, while body image was measured using a body image questionnaire that had been tested for validity and reliability. Data analysis was performed using the Chi-Square test with a significance level of α = 0.05. The results showed that the majority of respondents had Grade 2 and Grade 3 diabetic ulcers, and had a body image in the moderate category. The results of the Chi-Square test obtained a p-value of 0.000 (p < 0.05), which indicated a significant relationship between the degree of diabetic ulcers and changes in body image. Respondents with Grade 1 ulcers tended to have a positive body image, while respondents with Grade 4 and Grade 5 ulcers mostly had a less negative body image. The conclusion of this study is that there is a significant relationship between the severity of diabetic ulcers and changes in body image in patients with diabetes mellitus. Therefore, a holistic nursing approach that considers both the physical and psychological aspects of patients is necessary in the management of diabetic ulcers.
